Small Weight Tickets with Perforated Tab
This 9 × 4.25 weight ticket provides space to record the seller, buyer, date, driver, carrier, commodity, quantity and weight of shipped materials. The attached perforated tab gives you an additional record that can be separated from the main ticket.
Personalize the ticket with your company name and logo, and change the wording or fields to match your operation. It is available as a 2-part or 3-part NCR carbonless form with optional consecutive numbering.
