This 5.5×8.5 invoice is to be used inside a register machine that holds half-page forms. This particular general sales invoice includes customer order number, date, name, address and phone number all in one easy to read section. Below the customer information is where you record who sold the products or services and payment types. Payment types include cash, C.O.D., charge, on account, mdse. retd and paid out. There’s also an extra blank box for other types of payment. The table has four columns for quantity, description, price and amount. There are a total of 15 description lines for products, services or parts. An “all claims must accompany this bill” line has been provided at the bottom of the description lines. Under this line is where the consecutive numbering goes as well as the signature of the person who received the goods or services. The table ends with two lines for Tax and Total.
