Auto Repair Form Requirements for the State of Florida

  • Fee for the estimate.
  • Customer’s intended method of payment.
  • Name and telephone number of person, other than the customer, who will be able to authorize the repair.
  • The guarantee and its terms.
  • Section to indicate if the shop is to return parts to the customer.
  • Charge for daily storage if customer leaves vehicle with shop beyond three working days.
  • Disclosure statement.

The Repair Invoice Needs to Include

  • Auto shop’s name, address and telephone number.
  • Section to include the customer’s name, address and telephone number.
  • Date of when the vehicle was brought in for repair.
  • The proposed date that the work will be completed by.
  • Section to include vehicle information: year, make, model, odometer reading and license tag number.
  • Description of the work conducted.
  • Costs for labor, parts and other fees including waste removal.
  • Condition of the parts used indicate whether the parts are new, used or rebuilt.
  • The guarantee including its terms and conditions.
  • Certificate registration number of the auto shop issued by the Department.
