The top boxes are for the driver, their truck and trailer information, date shipped and client payment. The two large boxes make it easy to read shipper and consignee details. Twelve spaces have been provided to record items being shipped. The bottom of the form has two large boxes for dealer and driver notes. There are three lines of signatures for the shipper, consignee and driver.
