Form Requirements for Contractors in the State of Texas

Are you a licensed contractor in the State of Texas? If so, you need to provide important information on all of your business forms whether they are proposals, invoices or written contracts.

Contractor’s Information

In addition to adding the professional look to your custom printed forms, the following information is required by the State of Texas.

• Contractor’s name
• Address
• Phone Number
• License Number

The State of Texas requires the following verbiage on any proposals, invoices or written contracts for electricians:
