Auto Repair Form Requirements for the State of California

Before you design or pick out your forms from any print shop, you should be aware of the requirements set forth by the State of California. There are a few components that California’s Bureau of Automotive Repair (BAR) deems necessary to protect both the customers and auto repair shops. You have the option of providing your customer with an estimate form before providing them with an invoice OR you can use one form that will fulfill both duties.

Auto Repair Estimate Forms

If you will provide your customer with an estimate provided on its own specific form, make sure that the form includes the information below:

The auto repair shop’s name.
Auto repair shop’s address.
Phone number as well as any other contact information.
Auto Repair Dealer Registration Number (ARD Number) provided by the Bureau of Automotive Repair.
An estimate of the price for parts and labor.
Authorization for customer to sign allowing your shop to proceed with the actual work.

Teardown Estimate

If you need to conduct a teardown in order to provide your customer with an estimate, your form will need to include the following:

Notice that the vehicle may not be able to be re-assembled.
Cost of reassembling the vehicle, including the price of pieces needing to be replaced if they were damaged during the teardown.
How long the teardown will take.
Estimated cost to repair vehicle after the teardown.
Price of parts needed for actual repair.

Auto Repair Invoice Forms

Providing your customer with an invoice after the work is done? Make sure that the invoice includes the following sections:

The auto repair shop’s name.
Auto repair shop’s address.
Phone number as well as any other contact information.
Auto Repair Dealer Registration Number (ARD Number) provided by the Bureau of Automotive Repair.
Work performed including any work done at no charge because it is covered under a warranty.
List of parts replaced including their condition â€” new, reconditioned or rebuilt.
The cost of any labor completed.
Charges for the disposal of hazardous waste and a section providing your EPA number.
A check box indicating whether the parts are to be returned to the customer or not.

Guarantee

If you provide any type of guarantee for your customer, these specifics need to be known:

Name and address of business providing guarantee if other than your own.
What will the guarantee do? Will it repair or place parts OR refund money.
What steps customer must take to use the guarantee?
How long the guarantee lasts.
List any exclusions.
Note if the guarantee is prorated, based on time or mileage.
Can the guarantee be transferred to a new owner if the vehicle is sold?
