Order Form 752 is an 8.5x11 work order form. This form has areas to record the billing address, date order is placed, all the necessary contact information for the person placing the order and, plenty of room to record parts used in the job. This form was designed for jobs that require more room to list parts and their costs than most other forms. At the bottom of this work order invoice form there is an area to record, work ordered by, date ordered and date completed plus customer approval signature indicating the job was completed and the services rendered were satisfactory.
