This multi part carbonless repair form gives you full 8 lines to list the item and parts used in the service call with room to list the quantity and unit cost of each, and room to list the part name and description. This full page custom printed work order form has an area of recommendations, where the technician can write comments and recommendation on what needs to be done to the unit in the near future, the ideal place to suggest annual contracts to the client. We have even managed to include a release waiver at the bottom of this spacious form, authorizing the contractor to repair and test the equipment.
This multi part carbonless repair form is custom printed with your companies information and logo. We can even print a watermark of your company logo (provided that a proper logo is supplied and can be converted to 10% screen). Repair form 884 is an 8.5" x 11" and can be printed using 2, 3 and 4 part carbonless paper.
This full page repair form can be used as an estimate form or invoice form. Give one copy to the client for their record and keep another copy for yourself. If using a sub contractor, print this form in a 3 part carbonless paper so the sub contractor can keep a copy for their record.
